This is form is for you to request materials, equipment, supplies, or enrichments that you want Class Source to purchase for our program.

Please do not make any purchases without first requesting approval from the board via this form.

Remember that any monies or materials needed in order to for your class to function should be listed on the syllabus when submitted for approval BEFORE your class is added to the school year schedule. Any supplies used by students should be listed as well and those fees passed on the familles in the form of a curriculum or materials fee.

*Please note that Class Source is a 501.c.3 nonprofit and the Board of Directors must be consulted for any large purchase.
